ABSTRACT

Objective:To establish a BALB/c mouse model of Echinococcus granulosus allergy and investigate the role of lymphocyte subsets in Echinococcus granulosus-induced sensitization. Methods:Echinococcus granulosus was isolated from the liver of sheep naturally infected with Echinococcus granulosus and cultured for 40 d. BALB/c mice were intraperitoneally injected with 50 microcapsules and sensitized by intraperitoneal injection of 0.1 ml/10 g of larval Echinococcus granulosus capsule six months after infection. According to the symptom scores 1 h after sensitization, these mice were divided into two groups: non-sensitized group ( n=6) and sensitized group ( n=6). The mice ( n=6) in control group were injected with sterile saline. Blood sample was collected from the angular vein of each mouse. Flow cytometry was used to detect B cells, NK cells and CD3 +/CD4 +/CD8 + T cells. Cytometric bead array was used to measure IL-4, IL-6 and IL-13. Results:The percentage of B cells was significantly higher in the non-sensitized group than in the control group ( P<0.001), but no significant difference was observed between the sensitized group and the control group. Compared with the non-sensitized group, the percentage of B cells in the sensitized group decreased significantly ( P<0.01). Compared with the control group, the percentages of NK cells in the non-sensitized group and the sensitized group decreased significantly ( P<0.001 and P<0.01). Compared with the non-sensitized group, the percentage of NK cells in the sensitized group increased significantly ( P<0.05). Compared with the control group, the percentage of CD3 + and CD4 + T cells in the non-sensitized group decreased significantly ( P<0.05), while the percentage of CD8 + T cells increased significantly ( P<0.01). Compared with the control group, the percentage of CD3 + and CD4 + T cells in the sensitized group increased significantly ( P<0.05 and P<0.001), while no significant change in the percentage of CD8 + T cells was detected. Compared with the non-sensitized group, the percentage of CD3 + and CD4 + T cells in the sensitized group increased significantly ( P<0.05), while the percentage of CD8 + T cells decreased significantly ( P<0.01). The levels of IL-4, IL-6 and IL-13 were significantly higher in the non-sensitized group than in the control group ( P<0.001). Compared with the control group, the sensitized group showed increased IL-4 ( P>0.05), significantly increased IL-6 ( P<0.01) and decreased IL-13 ( P>0.05). The levels of IL-4, IL-6 and IL-13 in the sensitized group were significantly lower than those in the non-sensitized group ( P<0.001). Conclusions:The BALB/c mouse model of allergy induced by larval Echinococcus granulosus was successfully established. This study proved that the humoral immune response induced by Th2 cells played an important role in Echinococcus granulosus-induced sensitization, which provides an important scientific basis for establishing a prevention and treatment strategy for patients with anaphylactic shock caused by extravasation of Echinococcus granulosus fluid.

ABSTRACT

OBJECTIVE: To study the vasodilatory effect of oxysophocarpine (OSC) on isolated thoracic aortic rings of rats and its possible mechanism. METHODS: Thoracic aortic rings of rats were collected (called “vascular ring” for short). Using K-H nutrient solution as blank control and the diastolic rate as index, the effects of different concentrations (0.2-1.0 mg/mL) of OSC on normal vascular rings in basal state, normal or endothelium-free vascular rings pre-contracted by norepinephrine (PE, 1×10-6 mol/L) were investigated. After pre-culturing normal thoracic aortic rings by nitric oxide synthase inhibitor L-nitro-arginine methyl ester(L-NAME)and cyclooxygenase inhibitor indomethacin(INDO),as well as pre-culturing endothelium-free vascular rings by potassium ion channel blocker BaCl2,tetraethylammonium(TEA)and 4-aminopyridine(4-AP), the diastolic effects of OSC of different concentrations (0.2-1.0 mg/mL) on the above vascular rings were investigated by using the same method. RESULTS: Compared with blank control, there was no significant effects of different concentrations of OSC on the diastolic rate of normal vascular rings in basal state (P>0.05), but 0.4-1.0 mg/mL OSC could significantly improve the diastolic rate of normal or endothelium-free vascular rings pre-contracted by PE (P<0.01), in concentration-dependent manner. After preculturing with L-NAME, INDO, 4-AP and BaCl2, different concentrations of OSC had no significant effect on the diastolic rate of normal or endothelium-free vascular rings pre-contracted by PE (P>0.05). After pre-culturing with TEA and Gli, 0.4-1.0 mg/mL OSC could significantly reduce the diastolic rate of endothelium-free vas- cular rings pre-contracted by PE (P<0.01). CONCLUSIONS: OSC did not significantly dilate the thoracic aortic rings of rats in the basal state within the dose range (0.2-1.0 mg/mL), but OSC of 0.4-1.0 mg/mL have significant diastolic effects on the normal or endothelium-free thoracic aortic rings of rats pre-contracted with PE. The mechanism of thoracic aortic rings dilation is endothelium-independent, which may be associated with receptor operational calcium channel,Ca2+-activated potassium channels and ATP-sensitive potassium channels.

ABSTRACT

Objective@#Using propensity score matching method (PSM) to investigate the clinical effect of postoperative adjuvant radiotherapy and postoperative concurrent chemoradiotherapy for locally advanced hypopharyngeal squamous cell carcinoma.@*Methods@#From July 2007 to July 2018,174 postoperative patients with locally advanced hypopharyngeal squamous cell carcinoma were enrolled in pre-PSM cohort, including 168 males and 6 females, the median age was 60 years old (ranged from 37 to 79 years old).Loco-regional control (LRC),progression-free survival (PFS) and overall survival (OS) were compared and analyzed between the patients treated with postoperative adjuvant radiotherapy and postoperative concurrent chemoradiotherapy (cisplatin was given in a dose of 80 mg/m2 on days 1, 22, and 43). After the propensity score matching (PSM), 61 sub-pairs of 122 patients were generated in post-PSM cohort. Survival rate were assessed with Kaplan-Meier method and Log-rank test.@*Results@#After the propensity score matching(PSM), 61 sub-pairs of 122 patients were generated in post-PSM cohort. The patients were followed up for 3-135 months, the median follow-up was 42 months. No significant differences in 3-year and 5-year LRC, PFS, OS were observed between the two groups (P>0.05) . For postoperative patients who had high-risk factors (extracapsular extension of nodal disease, and/or vascular embolism, and/or lymph node metastasis≥2, and/or positive surgical margin, and/or perineural infiltration),there were significant differences between the two groups in 3-year PFS (60.99% vs 84.49%,P<0.05), 5-year PFS (35.47% vs 56.97%,P<0.05) and 5-year LRC (41.02% vs 68.50%, P<0.05), but no significant difference was found in OS between the two groups (P>0.05).@*Conclusion@#Postoperative concurrent chemoradiotherapy was more efficacious than postoperative radiotherapy alone in terms of loco-regional control and PFS for high-risk postoperative patients with locally advanced hypopharyngeal squamous cell carcinoma.

ABSTRACT

Objective@#To compare the treatment outcomes for locally advanced hypopharyngeal carcinoma between surgery plus radio(chemo) therapy(SRT) and non-surgery chemoradiotherapy(CRT).@*Methods@#A total of 119 patients diagnosed with advanced hypopharyngeal carcinoma without distant metastases between 2010 and 2014 were identified in the Chinese People′s Liberation Army General Hospital, and they were divided into 2 groups: 42 cases in SRT group and 77 cases in CRT group. Patients′ clinical information was collected. Survival rates and prognostic factors were analyzed by the Kaplan-Meier method with SPSS 23.0 software. The survival rates, laryngeal preservation rates and complication rates were compared between the two groups using the chi-square test.Among the 119 patients, 112 were males and 7 were females. Age ranged from 27 to 78 years, with an average age of 57 years.@*Results@#There were no significant difference between the SRT and CRT group for five-year disease-free survival (DFS, 53.9% vs. 45.1%, χ2=1.251, P=0.263) and overall survival (OS, 54.9% vs. 45.6%, χ2=1.749, P=0.186). Compared to SRT group, CRT group did not showed the significant increase of treatment complications (χ2=0.858, P=0.354), with a higher laryngeal preservation rate (50.0% vs. 71.4%, χ2=6.493, P=0.011).@*Conclusions@#Advanced hypopharyngeal carcinoma is of high malignancy and poor prognosis. Combined modality treatment is a main approach for advanced hypopharyngeal cancer. SRT offers disease-free survival and overall survival rates equivalent to CRT, but with a higher laryngeal preservation rate.

ABSTRACT

OBJECTIVE@#To explore and compare the effect of clinical characters on quality of life (QOL) of laryngeal carcinoma postoperative patients.@*METHOD@#We performed a retrospective cohort follow up study of 303 patients who underwent operation for laryngeal carcinoma. The Chinese version of the general and the head and neck specific quality of life questionnaires of the European Organization for the Research and Treatment of Cancer (EORTC QLQ-C30 and EORTC QLQ-H&H35) were used to measure the quality of life of those patients in the cohort, to study the relationship between QOL and clinical factors.@*RESULT@#In the 303 cases, there were 78 cases retaining the permanent tracheostomies (59 total laryngectomy and 19 partial laryngectomy); 4 patient retain the nasal feed pipe. Female patients were worse than male patients in the domain of physical function and global QOL (P<0.05). Older patients were worse than younger patients in the domain of physical function and speech (P<0.05). Patients with glottic tumors experience better global QOL than their peers with under glottic or supraglottic tumors. The patients with supraglottic tumor have more problem with swallowings (P<0.05). Earlier stage patients have better suitation in many domains than later stage patients, but III stage have more difficulty with swallowing than IV stage. Patients who accepted minimally invasive operation and laryngofissure have better QOL than those accepted partial laryngectomy or total laryngectomy; total laryngectomee have more problems with physical, emotion function, global QOL, sense, speech, cough and morbid feeling than patients with partial laryngectomy, but less problem with fatigue, pain, dyspnea, swollowing and dry mouth; Patients with partial horizontal laryngectomy have more difficulty with swallowing. In the multivariable analysis, the operation mode was an independent factor to speech problem. Patients who accepted neck dissction have more negative outcomes than patients without neck dissection. In the univariable analysis, the permanent tracheostomy was a notable factor which affected many domains in the quality of life; multivariable analysis showed that permanent tracheostomy was an important factor which worked on global quality of life.@*CONCLUSION@#The quality of life was affected by many clinical factors. The operation mode was an independent factor which affect speech problem. Decannulation after partial laryngectomy was very important to keep the quality of life.

ABSTRACT

OBJECTIVE@#To analysis the therapeutic procedure on the recurrent congenital fistula or cyst in lateral cervical part.@*METHOD@#Thirty-nine cases with recurrent congenital fistula or cyst in lateral cervical part were enrolled in this study including 12 cases from the first branchial cleft, 6 from the second branchial cleft and 21 from the third branchial cleft.@*RESULT@#All the cases underwent fistula or cyst excision for 2 to 5 times in their whole therapeutic process, not counting the incision and drainage. During 9 months to 17 years follow-up, fistula or cyst in 6 cases relapsed, including 1 fistula from the first branchial cleft,3 fistulae from the second branchial cleft, 1 fistula and 1 cyst from the third branchial cleft, respectively. One case with recurrent fistula from the first branchial cleft was diagnosed temporal verrucous carcinoma six months after the third fistula excision operation and died one year after the forth operation probably due to the intracranial metastasis of temporal bone verrucous carcinoma. In two cases, the fistulae went through the thyroid gland to the piriform fossa and both the fistulae and part of the thyroid glands were resected. In the patients whose inner orificium fistulae were found and ligated effectively,no recurrence occurred during the followed-up period.@*CONCLUSION@#The key point to cure the recurrent congenital fistula or cyst in lateral cervical part lies in proper occasion of operation, stain tracing in operation and reasonable program of operation.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the changes of NYGGF4 gene expression during human preadipocyte differentiation and to explore the regulative role of tumor necrosis factor alpha (TNFalpha) on NYGGF4 gene expression in matured human adipocytes.</p><p><b>METHODS</b>Human preadipocytes-visceral (HPA-v) were cultured in vitro and differentiated into the matured adipocytes. Fully differentiated adipocytes were treated with TNFalpha in different concentrations for 16 hours or at 10 ng/mL for various times. Levels of the NYGGF4 mRNA were evaluated by real-time reverse transcription-PCR(RT-PCR).</p><p><b>RESULTS</b>(1) After induction of differentiation, more than 90% of the HPAs-v exhibited typical adipocyte morphology on the 17th day. (2) In human preadipocytes, the level of NYGGF4 mRNA expression remained low. The NYGGF4 mRNA level was gradually increased with the preadipocytes being differentiated into the matured adipocytes, and reached the highest level in the fully differentiated adipocytes. (3) Treatment of the human matured adipocytes with TNFalpha resulted in a significant increase in the level of NYGGF4 mRNA. The up-regulatory effect of TNFalpha on the NYGGF4 gene expression tended to be enhanced with the increasing concentrations and elongation of time.</p><p><b>CONCLUSION</b>During human preadipocytes differentiation, the level of NYGGF4 gene mRNA expression increases gradually. TNFalpha could upregulate the expression of NYGGF4 mRNA in human matured adipocytes. The regulatory effect of TNFalpha on the NYGGF4 gene expression is of dose-dependent and time-correlated.</p>

ABSTRACT

Objective: To discuss the correlation between myeloid derived suppressor cells (MDSCs) and hepatic trans-planted tumor and to explore new ways to inhibit the development of hepatic cancer. Methods: We established the animal models with H_(22) hepatic carcinoma cells transplanted to the anterior right limb. Then the MDSCs morphology was observed with confocal microscopy and the proportion of MDSCs in blood and spleen was measured with flow cytometry. The 36 mice were divided into three groups: the control group, the low-dose group (2mg/kg) and the high-dose group (4mg/kg). Then As_2O_3 was injected twice a week to the mice before repeating the aforementioned measures. The direct effects of As_2O_3 on MDSCs cultured with H_(22)-ascites supernatant was observed. Results: At 25 days after transplantion, the tumor weight was increased to 5.67g, and the proportion of MDSCs in blood and spleen was increased to 20.46% and 9.50%, re-spectively. There was a positive correlation between hepatic transplanted tumor and MDSCs in blood and spleen and the relative factors were 0.95 and 0.96, respectively (t=-5.270 and 5.939, P<0.05). With the effect of As_2O_3, the proportion of MD-SCs in blood in low-dose group and high-dose group was 11.31% and 10.00% at 28 days after treatment, lower than that in the control group (t=3.193 and 5.486, P<0.05), and there was also a statistical difference between the high-dose group and low-dose group (t=3.066, P<0.05). The proportion of MDSCs in the spleen in low-dose group and high-dose group was 10.90% and 9.04% at 28 days, lower than that in the control group (t=3.586.and 5.279, P<0.05), but there was no statistical difference between the high-dose group and low-dose group (1=1.298, P>0.05). In vitro, the proportion of MDSCs in nutrient fluid was increased to 12.67% at 12 days after treatment with H_(22)-ascites supematant, and was decreased to 7.44% at 18 days after treatment with As_2O_3. Conclusion: The proportion of MDSCs in H_(22) tumor-bearing mice is increased because of tu-mor development. There is a positive correlation between MDSCs and hepatic transplanted tumor. As_2O_3 can decrease MD-SCs and inhibit tumor growth.

ABSTRACT

<p><b>BACKGROUND AND OBJECTIVE</b>The CIK cell is one of the most important means of the adoptive cellular immunotherapy, and it is a hotspot of which to simplify its culture procedure and to promote its inhibition rate. The aim of this study is to observe the biological function of the CIK cells cultivated by the recombinant human fibronectin (RN) and to establish an effective and simple way of cells expansion.</p><p><b>METHODS</b>We separated the mononuclear cells (PBMCs) in 50 mL peripheral blood from 10 healthy persons with density gradient centrifugation in the lymphocyte-separating medium, and the PBMCs were divided into two groups, of which were cultivated by RN-introduced and conventional method separately. Then we estimated the proliferation ability, and analyzed the immunologic type, IFN-gamma, IL-4, perforin and granzyme B of them with flow cytometry. Besides that, we tested the inhibition rate of CIKs cells to four kinds of human lung cancer cell lines in vitro by MTT assay.</p><p><b>RESULTS</b>The RN-induced group had a higher proliferation rate that was 2.0-3.5 times of the conventional group, and there was an obvious statistical difference between the two (P < 0.05). The proliferation rates of CD3+CD16+CD56+T cells in each group were 3 778 and 2 068 times of the initial number, respectively. There was also a higher percentage of CD3+CD8+ T cells in RN-induced group (P < 0.05), while the percentage of CD3+CD4+T cells had no significant statistical difference (P > 0.05). We found a similar inhibition rate of the CIK cells to all this human lung cancer cell lines (P > 0.05). The cells which secreted IFN-gamma increased, while the cells which secreted IL-4 did not. The cells which secreted granzyme B and perforin were positive.</p><p><b>CONCLUSION</b>It is an effective and simple way to cultivate the CIK cells with RN, which should be adopted.</p>

ABSTRACT

Objective:To evaluate the clinical effect of laryngofissure without tracheotomy.Method:One hundred and sixteen cases of laryngofissure without tracheotomy in our department were analyzed retrospectively.Result:Of 116 cases, 101 cases had the laryngeal carcinoma; 9 cases had laryngeal stenosis; 6 cases had benign tumor of larynx. All the incisions were healed by first intention; and postoperative duration of hospital stay were 7 to 20 days. Three cases recurred in 101 cases of laryngeal carcinoma (2.97%), and underwent reoperation, the 5 year survival rate was 100%. One case recurred in 9 cases of laryngeal stenosis, and underwent reoperarion of laryngoplasty with T-tube implanted. No recurrence in 6 cases of benign tumor of larynx. Conclusion:Laryngofissure without tracheotomy have the advantage of less trauma , fast rehabilitation and no destroying the normal respiratory functions. It is completely feasible so long as surgeons grasp the adaption of operation strictly and select suitable cases.

ABSTRACT

OBJECTIVE@#To evaluate the clinical effectiveness of injection n-Butyl cyanoacrylate (NBCA) in treating laryngopharynx hemangioma.@*METHOD@#Thirty cases of laryngopharynx hemangioma who received NBCA injection were obtained from our department 1998-2007. Twenty-five cases had tracheotomy under General anesthesia, and NBCA was injected into hemangioma by direct laryngoscopy. NBCA was mixed with iodide, and the concentration was 25.00% to 33.33%, NBCA dosage was 0.5 ml to 2.0 ml. There were 5 cases whose hemangioma confined to oropharynx didn't have tracheotomy, and they were injected straightly with the speculum oris. We observed the shedding of hemangioma in different time.@*RESULT@#Hemangioma with diameter less than 1 cm, shedded in about 1 month. However, hemangioma with diameter more than 4 cm, shedded in a period more than 3.5 month. No recurrence was observed in the follow up of 3 months to seven years.@*CONCLUSION@#Avoiding dissection of neck and repetitious operation, NBCA injection could be a safe, simple and effective therapy for laryngopharynx hemangioma.
